Before you submit this form, please ensure all information is completed. If you have more than one Value At Risk (VAR) you will need to fill out a separate form for each.

Carefully review all information entered in this form before submitting, as changes cannot be made after submission.

Details for the owner of the VAR

Provide a short, meaningful description that will let us know what Value we are looking for.

Provide contact information for you, the owner of the value, so you can be contacted in case a fire threatens your property or if we have further questions.

Geographic Location

Provide the decimal degree coordinates of the value AND a description. We must be able to locate the Value.

If you're unsure of the decimal degree coordinates of your Value at Risk, click the Google Maps link and follow the steps below. Google Maps

  1. Go to your property. Switching to satellite view (by clicking the layers icon in the bottom left) may help, though some structures might not be visible.
  2. Right-click on your Value At Risk.
  3. Left-click on the top line (the two numbers separated by a comma) to copy your location.
  4. Paste the copied location into the box below.
